Mastopexy without implants, or a breast lift, consists of repositioning the mammary gland in a higher position than the existing one, lifting the breast without increasing its volume. Excess skin and, in some cases, stretch marks are also removed.
In mastopexy without prostheses, our clinic uses the technique developed by Prof Ruth Graf, modified. What is the advantage of this mastopexy technique?
- It uses the droopiest part of the breast as an autoprosthesis, i.e. that part of the drooping breast is “utilised” and repositioned in the upper pole and attached to the pectoral muscle. The breast becomes more harmonious and rounded and less flattened.
- The scars are smaller than in traditional inverted T techniques, as in most cases you only get a circumareolar, vertical scar.

Type of Anaesthesia
Mastopexy without implants is performed under sedation and local anaesthetic. The first step is to administer the sedative intravenously so that the patient falls asleep, and then the local anaesthetics are injected. The sedation causes the patient to fall asleep throughout the surgery and the local anaesthetics lead to a loss of sensitivity to pain. The anaesthetist is present throughout the surgery for greater comfort and safety.
Type of Incision - Scar
Circumareolar scar
It involves an incision in the transition zone between the skin of the breast and the areola. It is indicated for milder cases of breast sagging.
Vertical Scar
It involves an incision around the areola and a vertical line from the areola to the inframammary fold. It is indicated for cases of moderate sagging.
T-inverted scar
It involves a circumareolar scar, a vertical line and a horizontal line in the inframammary fold. It is indicated for more severe cases of breast sagging.

Mastopexy step by step
Undergoing mastopexy surgery without prostheses is an important decision and as such, women should be aware of the surgical procedure and their recovery.
We applied the Rapid Recovery in all mastopexy surgeries without implants, allowing the patient to have a comfortable, short and mobile recovery hours after surgery.
The intervention follows these phases:
Bookings
The patient is marked standing up in order to correctly visualise the degree of breast sagging, the position of the areolas, existing asymmetries and the amount of skin to be removed.
Sedation
The sedative is administered through an injection by the anaesthetist who will be present throughout the surgery.
Disinfection and placement of surgical drapes
The breast and surrounding areas are disinfected to reduce the risk of infection. Sterile surgical drapes are then placed to isolate the intervention area from the external environment.
Local anaesthesia
Local anaesthetic is injected into the breast area. The patient will already be asleep, so she won't feel any stinging or pain during the surgery.
Incisions
Incisions are made according to the type of scar previously defined.
Breast tissue remodelling
The breast tissue is remodelled and repositioned to a higher position.
Areola remodelling
The areolas are repositioned in a more central position and, where necessary, their size is reduced.
Closing the incisions
The incisions are closed by planes to support the new breast structure. The stitches are resorbable.
Surgical glue
The glue is applied to the scars. It does not require dressings and gradually falls off from the second week.
Recovery
A post-surgical bra is put on and worn for 30 days. The patient is woken up by the anaesthetist and taken to recovery where she remains under observation until she is discharged.
Preparation for Mastopexy without Prostheses
Before Surgery
- Do not take any anticoagulant medication two weeks before surgery. On the day of your mastopexy appointment, you should inform the medical team of any medication you take regularly and any medical conditions you suffer from;
- Do not smoke before surgery and while the healing process is taking place in order to promote good healing;
- If you become ill, catch a cold or suffer from a bad mood in the days before the surgery, you should inform the medical team immediately;
- Carefully read all the documentation you've been given about mastopexy without prostheses, as some questions may arise;
- If you have babies or small children, you should have help at home during the first few days;
- Do you have any questions about the preparation or post-operative period for mastopexy without prostheses? Don't wait until the day of your surgery and ask all your questions first.
On Surgery Day
- The day before and the day of surgery you should eat light meals that do not cause indigestion. You should fast for 8 hours - do not eat or drink any liquid, including water;
- Bathe with the prescribed antiseptic gel the day before and the day of surgery;
- No jewellery or watches, no make-up or nail polish;
- Bring comfortable clothes to wear when you are discharged - preferably with buttons or zips to avoid lifting your arms;
- After discharge, you cannot go home alone or drive. If you don't have someone to drive you home, you must inform us before the day of the operation.
Post-operative care
- The outcome of the surgery is directly related to post-operative care, so it is extremely important to follow all the doctor's instructions to the letter:
- Take the prescribed medication;
- Do not drive for 8 days;
- Wear the surgical bra for 30 days;
- Use the band for 15 days;
- Comply with the physiotherapy programme (lymphatic drainage);
- Return to work is expected after one to two weeks (sedentary professions);
- Avoid smoking;
- Drink plenty of water and eat a balanced diet;
- Attend all review appointments.
- If you live more than 60 minutes' drive from Lisbon, you must stay one night at the clinic or at a nearby accommodation accompanied by a relative. We'll be happy to help you organise travel and accommodation.

Whilst it's true that the majority of women go through surgery extremely comfortably, it's still necessary to allow time for a smooth recovery, so as not to precipitate unnecessary complications. O full recovery time of mastopexy without prostheses always ends up going for at least 4 weeks.
At an early stage, there is a lot of oedema and you shouldn't exert yourself physically for the first 4 weeks to reduce the risk of haematomas and seromas. Wanting to rush recovery can be counterproductive and end up causing more complications in the short/medium term.

Mastopexy does not stop the natural ageing process. As we get older, the breast continues to undergo changes due to the normal loss of skin elasticity, weight fluctuations and future pregnancies/breastfeeding. However, it will always start from a better level. We recommend maintaining your weight, eating healthily and exercising for longer-lasting results.
How long can I wear a bra with underwires?
For the first 30 days, you should always wear a surgical bra. After this time, you can wear a bra with underwires whenever you feel comfortable.
Will the scar disappear?
Scars don't disappear, but over time they become thinner and lighter in colour. The final result of a scar is expected after 12/18 months and its final appearance depends on various factors such as the patient's genetic characteristics and post-operative care.
Will I need drains?
The use of drains is of great importance as they help to eliminate the fluids that accumulate inside the breast after surgery. They are placed whenever necessary and are usually removed after 24/48 hours.
