Slim waist: learn about costal remodelling
Costal remodelling is an aesthetic procedure that aims to modify and reposition the false and floating ribs, the last three ribs of the rib cage. A alteration of the costal curvature makes it possible to achieve a slimmer, more defined waist.
In this article, we clearly explain what it is, who it's for, how it works and what to expect post-operatively - so you can make an informed decision.
Where does the desire for a slim waist come from?
The slim waist is an aesthetic ideal with historical roots - from the corsets of the 19th century to the valued silhouettes of the 50s. Today, social media and public figures continue to reinforce this standard of a feminine silhouette with a well-defined waist.
According to a study published in Aesthetic Surgery Journal (2023), the demand for waist-thinning surgeries - such as costal remodelling - has increased by 30% in the US in the last five years.
Costal remodelling is yet another a minimally invasive surgical procedure that can help you achieve a more defined waistline, particularly in patients with more prominent last ribs, or when other methods have not been sufficient, such as liposuction. Like any surgical procedure, it requires rigorous assessment and medical follow-up.
What is costal remodelling?
Costal remodelling involves the modification of the last ribs, which are usually the floating ones (11th and 12th) and, in some cases, the 10th. . By acting directly on the bone structure, this technique makes it possible to achieve a slimmer, more defined silhouette - something that is often not possible with diet, exercise or liposuction alone.
The surgery is performed with the patient in the prone position (face down) and under general anaesthesia. The procedure lasts about an hour and is made through a small millimetre incision strategically positioned over each of the ribs to be remodelled, in order to ensure that the scars are as discreet as possible.
It is a precise and safe technique that allows permanent and harmonious results to be achieved, always with respect for each patient's individual anatomy.
Myth or reality? Clarify the most common doubts
“Is it effective?”
When carried out properly, in well-selected patients and in compliance with post-operative indications, it allows an average of 10 cm of waist measurement to be permanently reduced.
"Is it final?"
Yes, the results are permanent. However, they can be influenced by variations in weight and lifestyle. If you gain weight and localised fat in your abdomen after the procedure, you may not see such a significant reduction in your waist measurement.
"Is it safe?"
This is one of the most common and legitimate concerns. Rib remodelling is considered very safe when performed by qualified and experienced surgeons. The surgeon locates the ribs to be modelled using ultrasound. A small millimetre incision is made on each of the ribs to be modelled in the posterior region of the torso. The rib is “weakened” in the incision area with a piezoelectric ultrasound device (PIEZO). This surgical device, also used in rhinoplasties, allows high-precision bone surgery with maximum safety.
"Does it affect the organs?"
No. The procedure only works on the ribs, and vital organs are protected. PIEZO works only on the bone and has no cutting or piercing effect on soft tissue, which allows us to preserve and avoid injury to the structures surrounding the ribs (blood vessels, nerves or lungs).
"Does it leave scars?"
The minimally invasive technique uses just one millimetre incision on each rib to be reshaped, in the back area. As well as being very small, the incisions are positioned in a very discreet area.
"Do you feel pain?"
There may be some discomfort in the post-operative period, but it is usually mild and well tolerated. With appropriate medication and compliance with medical recommendations, pain is effectively controlled.
These answers help to dispel common doubts, but it's essential to discuss everything with your surgeon to receive personalised information.
Costal remodelling: who can have the procedure?
Costal remodelling is indicated for healthy adults with a stable weight and compatible body structure who wish to slim their waist or improve their chest contour. It can be an option for those with rib asymmetries or who want additional refinement after other body treatments.
More than an aesthetic desire, it is essential to have solid personal motivation, realistic expectations and emotional stability. This procedure is not recommended for people with significant or uncontrolled chronic illnesses (such as heart or respiratory problems) or active smokers, due to the increased risks.
In the initial medical assessment, The patient's clinical history, anatomy, motivations, expectations and level of commitment to the post-operative period are analysed. It should also be emphasised that this surgery is no substitute for a healthy lifestyle - a balanced diet and regular exercise are still essential to maintain the results.

Recovery and results: what to expect?
The recovery period is usually one week, but it can vary from person to person. However The final result is stabilised after 3 to 6 months.
The use of a progressive compression belt is essential during this period, helping to shape the new silhouette and reduce swelling.
Initially the tightness of the girdle should be increased daily, from the first day after surgery, in order to reduce the waist measurement by 1cm per day. When the desired size has been achieved, the same degree of belt tightening should be maintained for the following 3 months.
Proper post-operative care - such as avoiding physical exertion, wearing a girdle and maintaining a healthy diet - are fundamental to the success of the surgery.
